As one of the most prominent medical institutions in the world, UCLA Health has always supported the use of instrument/product/equipment evaluations to advance clinical knowledge and patient care. These evaluations provide our physicians the opportunity to assess new, emerging medical products that would otherwise go unnoticed. UCLA Health dedicates itself on innovation and progress, and instrument/product/equipment evaluations play a vital role in this commitment.
Procurement and Strategic Sourcing has developed an evaluation process where requesting physicians may submit their requests to trial these new items. An evaluation request will first be reviewed by a Procurement lead. Once initially approved, the request must then be approved by the department manager and Senior Administrative Leadership. At the very end of the evaluation approval track, the requesting physician will receive an acknowledgment email.
ALL Evaluation Requests MUST be reviewed PRIOR to the start date by Procurement & Strategic Sourcing:
Budget-Responsible Managers/Unit Directors who receive evaluation requests for non-formulary products that are no-charge or <$10,000 can be approved by the appropriate Manager.
Evaluation requests in excess of $10,000 will be reviewed by appropriate Procurement and Strategic Sourcing individual listed above, appropriate Manager/Department Head/Department Chief AND Senior Administrative Leadership PRIOR to the evaluation start date.
All efforts should be used to obtain instruments/products/equipment on a no-charge basis. If approved, a no-charge PO will need to be completed PRIOR to the evaluation start date.
If consumables are involved at a cost, one-time EMPAC and EAP numbers will be created for use during the evaluation period. An EMPAC Item Add request form and spreadsheet will need to be submitted by the requestor if approved.

IMPORTANT: Note that there is no save feature in the Evaluation Request Portal. Prior to beginning your form submission, please make sure you have all the necessary information and documentation. Requestors must also reattach their files to the form, if an error occurred during original submission. You must be connected to the UCLA network (intranet) to access the Evaluation Request forms.
